IRS fax cover sheet: what to put on it

We couldn't find a general IRS fax cover sheet, so use a plain one with your name, phone number, the form you're sending and the page count. Leave your Social Security number and EIN off it. The IRS's own cover sheet lists for faxing some tax forms say not to put sensitive information on the cover sheet.

When you send a fax from faxfair.com, these cover sheets are free to add, and your business logo on one costs $0.99. The logo prints in black and white, like the rest of the fax. The PDF and Word downloads don't have a logo option.

What to put on an IRS fax cover sheet

On its pages about faxing Forms 8886, 8918 and 8023, the IRS lists what to put on the cover sheet. Using the same list for other IRS faxes is our suggestion, not an IRS rule.

Find the right fax number

We found no single IRS fax number. Each form's instructions give their own, and for most forms the number depends on where you live. Use the instructions for your form, and check them on the day you send, because the IRS says these numbers can change.
